Obituary for Philip David O'Leary
Philip David O'Leary was born and entered into rest on April 20, 2015. He is survived by his parents Kirsten and Daniel O'Leary, his sisters Shiloh (6), Abagail (4 1/2), brother Benjamin (1 1/2), maternal grandparents Scott and Lynn Altmayer of Pittsburgh and paternal grandparents James and Laura O'Leary as well as many aunts, uncles and cousins.
He was preceded by his brother Jedediah.
The family will receive friends from 1 p.m .to 2 p.m. Saturday at Life House Church, 14030 Radcliffe Rd., Chardon, Ohio. A funeral service will follow at 2 p.m. with Rev. Evan Nunnally officiating.
Memorial contributions are suggested in care of the family.
Arrangements by Johnson Funeral Home of Painesville.
